Bright red or inflamed gills can be caused by ammonia or nitrite exposure, external parasites, bacterial gill disease, injury or other irritation. Some fish naturally show visible red gill tissue, so compare with their normal appearance. If redness is new and accompanied by fast breathing, gasping or lethargy, water testing and oxygenation are the first priorities.
Gill problems can become serious quickly because fish rely on a thin, delicate respiratory surface. The best first test is often not a medication—it is the aquarium water.

Environmental causes to rule out first
- Ammonia above zero in a stocked tank.
- Nitrite above zero.
- Very low dissolved oxygen.
- Sudden pH or temperature change.
- Chemical contamination or untreated tap water.
Parasites and infection
Gill flukes and other external parasites can cause irritation, excess mucus and rapid breathing. Bacterial gill disease and columnaris can also damage gill tissue.
Microscopic examination is far more reliable than guessing from colour alone.
Look at breathing as well as colour
Rapid opercular movement, surface gasping or crowding near an outlet indicates respiratory stress. Increase aeration and correct water quality while investigating the cause.
A fish with normal breathing and naturally visible red gill tissue may not be ill.
What to do with a positive ammonia or nitrite test
Use conditioned partial water changes, verify the filter is running and stop adding fish. Review whether the biofilter has been disrupted or overloaded.
Retest until the readings stay at zero.
When veterinary help matters
Persistent respiratory distress with normal water quality, visible gill lesions or multiple deaths justifies diagnostic help. Gill disease can worsen rapidly and may need targeted treatment.
